The Post Holder will be responsible for monitoring the physical health of patients within Cygnet Hospital Bury, working closely with the Lead Physical Health Nurse & Clinical Quality & Compliance Lead.
The role will involve co-ordinating the GP clinic on a weekly basis, taking responsibility for referring patients if other health professional involvement is indicated, ensuring that documentation is completed i.e. GP notes, physical health monitoring forms and Physical Health Care Plans. The role will involve liaising with the teams on each clinical area, responding to the educational needs of patients and becoming involved in the training of staff as required.
• Newly Qualified or an Experienced RGN / Physical Health Nurse with a current Pin Number
• Have experience working with an adult mental health client group
• Have knowledge of the Mental Health Act 1983, an understanding of latest nursing practices.
• Have the capacity to maintain documentation and undertake audits
• Compassionate and focused on patient recovery to monitor, manage and reduce risk whilst helping to empower and support service user independence.
